Course Outcome
This assignment enables the student to meet the following Course Outcome CO:
CO 1: Apply evidence-based leadership skills and concepts in the execution and evaluation of an executive-level practice change project. (PO 4,5)
Due Date: MT at the end of Week 6
Students are given the opportunity to request an extension on assignments for emergent situations. Supporting documentation must be submitted to the assigned faculty. If the student’s request is not approved, the assignment is graded and a late penalty is applied as follows:
Monday = 10% of total possible point reduction
Tuesday = 20% of total possible point reduction
Wednesday = 30% of total possible point reduction

Project Transition Plan
The ideal situation is that the project manager dedicated to a specific project should oversee it until completion. However, this is not always the case because of various underlying issues. Commonly, project managers transition from one project to another, leaving a different person to take over the previous assignment. The present scenario is a case of such a transition in a healthcare project where the current manager has to supervise a new project. When such cases occur, the current project manager should prepare adequately to hand over the project to the new manager so that the project continues without undue disruptions.
Activities in the Final Week
The current project manager should prepare a project transition plan with detailed information about what the incoming project manager should know about the project. The process involves the preparation to introduce the incoming project manager to the project. During the final week of transition, the current manager should hold regular meetings with the incoming to discuss the project scope and timelines, budget, deliverables, and stakeholders. The manager will also meet the current project team to discuss the transition and introduce them to the incoming project manager (Harrison & Lock, 2017). The meetings will also involve discussing progress, the current status, and expectations. The current manager will also update the new project manager with the most recent information to avoid transferring challenges. Notably, the leader should also discuss the change with the customer and explain the reasons behind the process.
Qualities of the New Project Manager
The new project manager should have the project’s background information to deliver specific deliverables. Besides understanding the project, the new project manager should effectively communicate expectations, goals, and responsibilities with team members. The potential manager should also have strong leadership skills to lead the team. Notably, the leader should motivate team members to maintain the morale they had before the change. Conflict resolution and problem-solving skills are others areas of competence that the new project manager should demonstrate to lead the team successfully. For instance, considering the diversity in the team, skills in conflict resolution are some of the most important proficiencies. Novo et al. (2017) aver that new project managers should exhibit team building and delegation skills to ensure that members meet their individual and team objectives. Hence, such characteristics will ease the transition and ensure that the project continues unabated.
Ensuring the project will Achieve the Practice Change
Effective project management involves preparing for the change at any point in the lifecycle. To ensure success in the change process, the project manager should integrate change management and project management from the project’s initiation phase. Another critical factor in achieving success during the change is connecting the people and the transition’s technical side (designing, developing, and delivering the solution). The manager should inform project team members about the potential change to avoid surprising them, which can be counterproductive. According to Verenych et al. (2017), a transition plan and other relevant documents should always be ready to ensure a smooth change. Generally, the manager should prepare the structures, processes, and tools to implement the change.
Conclusion
As is evident from the discussion, project managers should always prepare for change or transition in project management. To ensure continuity, they should have a specific plan to hand over the project to a new project manager. Besides a transition plan, they should prepare stakeholders for a potential change. Overall, the process is necessary and should have support from all stakeholders to guarantee a successful transition.
