This is a two part assignment I would like the same person chosen for this assignment for the second part as well. You will need this assignment to do the second part of the assignment it goes together. The person will also have to have access to this book due to having to use it as one of the sources. Weber, J. R., & Kelley, J. H. (2018). Health assessment in nursing (6th ed.). Wolters Kluwer. For the rest of the instructions I will have attach it below. Please use the rubric in its entirety if you have any questions please let me know. Thank you!
Patient Teaching Plan | |
HEALTH TOPIC | ANSWER |
State the topic you have selected for your Teaching Project. (Please select from the list provided in the Teaching Project guidelines located in Module 4.)
Describe in detail why this is an important topic for patient education. Use evidence from the textbook, lesson or an outside scholarly source to support your rationale. |
The topic of focus for this teaching project is medication safety at home.
Typically, medication is the primary treatment intervention for the majority of patients diagnosed with illnesses in healthcare facilities. It is estimated that approximately 6,800 prescription medications and several over-the-counter drugs are available in the United States (Tariq, Vashisht & Scherbak, 2020). Often, some of the medicines are prescribed to patients for self-administration after being released from the hospital. However, studies reveal that medication errors can occur, either due to prescribing, dispensing, and drug administration errors, and that patients can be responsible for some of these errors (Sequera, 2015). Given that self-administration medication is a common treatment intervention, it is vital to educate patients about medication safety at home. Such a teaching project can help prevent patient harm that may arise from medication use and reduce readmission rates and high healthcare costs associated with a lack of medication safety at home. |
POPULATION | ANSWER |
Describe, in detail, the characteristics of the population you are planning to teach with the Visual Teaching Tool.
|
The target population for the teaching project is elderly patients, both male, and female, that are above the age of 65 years. Most notably, this population is vulnerable to poorer health, have more chronic disabilities, and function less independently, mainly because of the physiological changes of aging (Weber & Kelley, 2015). Therefore, they may be subject to frequent medication, some of which require self-administration after being discharged from the hospital. |
BARRIERS | ANSWER |
What are some potential learning barriers for this population of learners? (Barriers might be cultural, physical, educational, or environmental. Refer to the assigned article in the project guidelines for more information.) |
The most potential learning barrier for the elderly population is physiological factors.
· The elderly population is known to have lower visual and auditory acuity (Beagley, 2011). Therefore, clearer images and graphics, and louder higher-pitched sounds will be utilized to enhance the acuity of the visuals and auditory for the population. Also, a quite setting will be used to improve the hearing acuity of the group. · The target population is also known to have short-term memory loss and may experience challenges in processing multiple simultaneous messages (Beagley, 2011). As such, short learning sessions, coupled with simple and straightforward information, will be utilized and backed with handouts with pictures for patients to take home to ensure that they have a better understanding of medication safety. · The target population is also known to have a lower capacity for drawing conclusions from inferences (Beagley, 2011). Therefore, specific directions and quantities will be utilized to ensure that they comprehend the particular manner in which to store medication, the amount that should be administered, and the time of the day of medicine administration. |
Describe how you could develop your Visual Teaching Tool in a way that will address these potential barriers.
|
· Larger and more clearer images and graphics could be used in the visual teaching tool to enhance visual acuity for the elderly population.
· Quantifiable and specific instructions could be used in the visual teaching tool to help the patient overcome the barrier of concluding inferences. |
SETTING | ANSWER |
Where do you plan to utilize your Visual Teaching Tool? (Examples: primary care clinic, health fair, school, etc…) |
I plan to use the visual teaching tool in a primary care clinic. Most notably, patients’ first contact of care mainly begins in the primary care setting. As such, this will be the best setting to enlighten the target population about medication safety at home. |
Will you be teaching one-on-one, in small groups, or to a large crowd?
|
Teaching will be done on a one-on-one basis. This approach of education is selected because it is likely that each patient has a preference for a different teaching style. Therefore, it will be vital to teach each patient separately using a technique that meets their level of satisfaction. |
LEARNING OBJECTIVES | ANSWER |
Write three specific learning objectives your visual teaching tool will address. Example: At the end of this education, the learner will be able to list 3 benefits of regular physical activity. |
1. At the end of this education, the learner will be able to define medication safety. |
2. At the end of this education, the learner will be able to list at least three strategies of keeping medicine safe at home.
3. At the end of this education, the learner will be able to demonstrate how to administer and store medicine safely at home.
EVALUATION | ANSWER |
Write a paragraph describing how you could evaluate whether your visual teaching tool was successful and met the learning objectives. Consider the population’s abilities and the setting.
|
Evaluation is an essential step in a teaching project as it helps educators assess the effectiveness of their projects. In this context, I could evaluate the efficacy and success of my visual teaching tool in meeting the learning objectives by monitoring the reaction of the learners to the tool. Most notably, a negative response could imply that the tool did not meet the learning objectives. Also, I could request the learners to recall the knowledge gained based on images exhibited on the visual tool to test its effectiveness. Most notably, the ability of learners to recall the knowledge by only looking at the visuals would imply that the tool was successful in meeting the learning objectives. |
REFERENCES | ANSWER |
List any references used to create this Teaching Plan in APA format. (Hanging indent not required.) Remember to also use in-text citations within this document, when appropriate (Author, year).
